​This form is only to be used by those organisations that have been undertaking work, outdoor recreation, or citizen science activities outside of urban environments, and have been specifically requested by NRE Tas to report their activities. 

If you need to report sick or dead birds or other animals:

• Avoid contact (do not touch, and keep pets away)
• Record what you see (photo, video, location, detail)
• Report it as soon as possible to the Emergency Animal Disease Hotline 1800 675 888 (24/7), or through our online reporting forms.
